Repair Requests
Please complete the below form to submit a repair request. You will be able to select the area that you would like assistance with. If you would like to attach a picture to the request, make sure to take the picture before opening the request.
Room Changes
If you are interested in changing rooms, please complete the form and submit it using the link: https://wongjai22.wixsite.com/housing-website/room-change-request We will then review your request and check with any other students involved to ask their feelings on the request. You will receive a reply email regarding the status of your request.
Check-Out Request
Once you have completed your studies or plan to leave to collect data, you can request to check out of your room. Requests must be submitted ONE MONTH before you plan to leave, otherwise, you will not receive any of your housing deposit back. Fill in and submit the form to foncmuhousing@gmail.com. You will receive a reply email regarding the status of your request.
Traveling Outside of Thailand Notice
To request to travel outside of Thailand, please complete and submit the information in your Visa Service Profile: https://app.nurse.cmu.ac.th/overseavisa/Visa/Login
Housing Contracts
Below, you will find a copy of the housing contract.
Once you arrive, you will be asked to sign a copy of the housing contract and pay the security deposit.
Replacement Item Costs
During any time, but especially the check-out process, if there are any items that the dorm staff deems unfit to use for future residents, the cost of replacing such items will be taken from your security deposit. If there are additional charges over the security deposit, you will be required to pay out of pocket.
